What Is Included?

• Treatment for the upper or lower 6 front teeth only.
• Treatment can be single crowns, bridges, veneers and/or a combination.
• A consultation with a doctor for diagnosis.
• Complete Dental prophylaxis ( dental cleaning).
• X-rays of proposed treatment area.
• Preoperative diagnostic wax-up of the selected  teeth.
• Three to five total appointments required.

Additional Important Information:

• This package price does not include the treatment of decay, abscesses or any other tooth      problems.  
• If additional procedures are needed, they will be quoted at our normal fees.  
• These same guidelines apply to any additional procedures such as restorations under crowns, additional selected crowns added to makeover treatment and preoperative wax up to include additional crowns.  
• There is no warranty for this treatment. 
• A 20% non-refundable deposit is needed to schedule the treatment appointment. The remaining balance is due in full the day treatment is started.
